Due to the ever changing insurance industry and multiple plans offered by each insurance carrier, it is best to confirm The Steadman Clinic’s participation by calling your insurance company. Please provide them with our Tax ID number 84-1415470, and physical address: 181 West Meadow Drive, Suite 400 Vail, CO 81657. Each of our physicians are listed under that Tax ID number.

Many insurance carriers require that we collect co-pays and/or deductibles from the patient. If you have an office visit co-pay for a specialist, you will be expected to pay that amount at the time of service. Due to federal laws, as well as our individual contracts with the insurance carriers we are prohibited from waiving these fees.

  • Anthem BCBS PPO: Pathway Essentials, Plans that have no out of state benefits or have IFM waiver requirements that are not completed by the patient

  • Cigna: CO EPO Connect, Medicare Advantage PPO* (Likely has out of network benefits that can be utilized)

Worker’s Compensation

For patients whose injury is work related, you must provide the schedulers with all pertinent information such as insurance carrier, claim number, date of injury, adjustors name, phone and fax number before the appointment is scheduled.

We may be unable to accept Workers Compensation Claims that occurred outside of the state of Colorado
 
Other Insurance Carriers

If we are not contracted with an insurance plan, it is our policy to collect 100% of the bill at time of service. As a courtesy, we will submit the claims to the insurance carrier on your behalf. If the carrier does not pay on the claim, you will be contacted for the remaining payment on your account.

Workers Compensation

Referrals

A referral is sometimes required in order to be seen by a specialist. You will need to contact your insurance company to find out if a referral is needed. You must ensure that we have the referral prior to your visit, otherwise you will be financially responsible at the time of service.
